Part time Registered Nurse Case Manager Anticipated TerritorySacramento County
ScheduleEvery Saturday and Sunday full days. Consider Saturday/Sunday plus 3 weekdays.
What you’ll do:- Conduct Start of Care (SOC) admissions – OASIS
- Conduct follow‑ups and evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to the Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and administer IV medication infusions safely in accordance with established protocols
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
- Flexible work schedules: create your own schedule and hours
- Targeted territories within a 5–15 mile radius

- Current and unrestricted California RN license
- Current BLS/CPR certification
- Valid California driver’s license
- Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired
- High level of clinical knowledge in home health care highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team highly desired
- Case management fundamentals highly desired
- Previous experience with IV medication infusions highly preferred
- Proficiency in initiating intravenous (IV) therapy is a plus
