Position Overview

Clinical Case Managers on the Re-entry Treatment Team have a unique ability to relate to and motivate clients due to their personal shared reentry experience or closeness with loved ones who have had this experience. They understand how to navigate the distinct challenges that returning citizens face using approaches informed by a strong working knowledge of trauma-informed clinical interventions and community-based resources in Alameda County.

Clinical Case Managers focus on providing intensive case management, prioritizing strong collaboration with the clinical team and supporting clients with engagement in services consistently, accompanying clients to appointments, and ensuring they receive necessary services. They will also refer and link participants to housing, employment, and health-related resources in the community. This Clinical Case Manager will be skilled in providing services which result in clients "dealing more effectively with daily problems," promoting engagement, independence and community reintegration.

The RTT Clinical Case Manager reports to the assigned Program Supervisor/ Manager, and works closely with RTT clinicians and trainees, Intake/ Program Coordinator, psychiatry providers, probation officers, county partners, housing providers and other community agencies to coordinate comprehensive client care.

This position is covered under the SEIU 1021 Collective Bargaining Agreement. In addition to the established base pay, members will receive negotiated annual increases as follows:

  • January 2027: 3% base pay increase

Future increases will be subject to the terms of the collective bargaining agreement.

Essential Job Functions And Responsibilities:

  • Provide intensive care coordination/case management services for justice-involved adults with serious mental illness (SMI), utilizing recovery-oriented and trauma-informed practices that connect clients to resources, employment support, referrals for psychiatric treatment, linkage to mental health, primary care provider, substance abuse, legal, and life skills services.
  • Conduct community-based outreach and engagement activities to establish rapport, promote treatment participation, reduce barriers to care, and support successful community reintegration.
  • Monitor client mental health symptoms, behavioral health needs, and risk during community visits and appointments; communicate significant concerns to the treatment team and participate in crisis intervention and conduct safety planning as appropriate.
  • Assist clients in increasing motivation to engage in mental health treatment, psychiatric services, medication support and substance use recovery services.
  • Support clients in developing organizational, independent living, and self advocacy to improve functioning and long-term stability.
  • Coordinate access to medical, dental,psychiatric, substance use, and other health care services and supports while promoting continuity of care.
  • Assist clients in navigating, obtaining and maintaining appropriate stable housing, including emergency local shelters, board and care homes, transitional housing and independent living placements in the community by sharing resources and supporting application processes where indicated.
  • Coordinate intake appointments including processing medical eligibility checks and tracking timeliness of first and follow up appointments.
  • Coordinate referrals and linkage to employment, vocational training, educational opportunities, legal services, transportation resources (bus pass, gas card, etc..), food assistance, and other community-based supports.
  • Serve as role model and coach to inspire, engage, and advocate for participants and their family member/caregivers' rights as they navigate the criminal justice and court systems.
