FAMILY INTERVENTION CASE MANAGER
Full-Time / Salaried / Justice Works Youth Care

Schedule: Full-time with some evenings/weekends
Location: Luzerne County, PA
Pay Range: $35,000-$40,000

About the Role

Justice Works Youth Care is hiring a Family Intervention Case Manager to support children and families through assessment, advocacy, and service coordination. In this role, you will work as part of a multidisciplinary team to promote child safety and family stability while collaborating with community partners such as social workers, psychologists, and educators. This position is ideal for individuals passionate about Child Welfare, Juvenile Justice, or Social Work.

Minimum Requirements
  • High school diploma required
  • Valid driver's license, auto insurance, and reliable access to a vehicle
  • Ability to pass all required background clearances
  • Ability to pass pre-employment, post-offer drug screening
  • Willingness to work non-traditional hours, including evenings and weekends
What You'll Do
  • Assess and engage families in service planning based on identified needs
  • Plan and implement appropriate interventions using evidence-based and best-practice approaches
  • Provide advocacy and connect families with community resources
  • Collaborate with internal teams and external professionals to support family goals
  • Transport clients using your personal vehicle as necessary
  • Maintain accurate documentation and meet program requirements
