The Social Services Assistant is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

• Meeting the resident/guest on or shortly after the admission and following them until they are discharged to the community.
• Attend the 72-hour care plan meeting.
• Confirm contact information on the face sheet.
• Ordering or recommending resources to acquire durable medical equipment as needed/recommended by therapy for mobility or nursing to attain optimal health status and ordered by practitioner.
• Arrange home health care for after discharge with the resident/guest's preferred provider, as soon as possible.
• Ordering and coordinating IV home infusion services, wound vacs, trach supplies, tube feeding supplies, vents and vent supplies, if necessary.
• Coordinating nursing education with the proposed caregiver.
• Overall coordination of the discharge.
• Attending care conferences.
• Attending Medicare Meetings.
• Any item related to the discharge process for residents who will be going home.
• Ensuring completion of the Post Discharge Plan of Care and Summary.

• High school diploma/GED.
