Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments, documenting findings and coordinating follow-up with PCPs. You will review medical history, medications and risk factors to close gaps in care during each visit.
The role requires NP licensure in the practicing state, board certification in an Adult/Geriatric or Family specialty, and the ability to travel extensively. A strong computer skill set and excellent communication are essential.
