About the PositionThe Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families. The DDP Targeted Case Manager conducts comprehensive assessments and periodic reassessments of individual needs, develops and periodically revises plans of care, refers and coordinates services to help members obtain needed supports, monitors outcomes, follows up, and ensures the quality of each plan of care.

Qualifications- Above average ability to follow written and verbal directions
- Strong oral and written communication skills
- Above average ability to recognize, analyze, and solve complex problems/dilemmas
- Ability to facilitate, negotiate, mediate issues, and promote team consensus
- Bachelor’s degree in social work or a related field from an accredited college
- Minimum two years of human services experience, with at least one year in developmental disabilities (DD). If lacking one year, the candidate must complete at least 40 hours of DD training within three months of hire
Conduct comprehensive and periodic reassessments, develop and revise plans of care, refer and coordinate services, monitor service delivery, follow up with clients, and ensure the quality of each care plan.

- Statutory $1.00 hourly base pay increase effective July 1 2026
- Primary location open for consideration in Helena or Butte; specific conditions will be provided by the supervisor
- Position remains open until filled, with frequent screening of applicants
- Training assignment may be used; employees may be paid below base pay per agency policy
$27.55 per hour. Telework Eligible (telework schedule must be supervisor approved). Benefits:
