Case Manager

• Serve as the primary liaison between families, guardians, community partners, and the treatment team, ensuring clear communication and coordination throughout treatment
• Coordinate referrals, assessments, admissions, treatment planning, case conferences, and discharge planning.
• Collaborate with families, providers, and community agencies to support treatment goals and address barriers to progress.
• Provide families with education, resources, and support to strengthen engagement and improve outcomes for children.
• Maintain accurate, timely documentation and ensure compliance with agency, Medicaid, and regulatory requirements.
• Monitor treatment progress, facilitate ongoing review of care plans, and support continuity of services throughout the child's treatment journey.

Required

• Graduation from an accredited college or university with a bachelor's degree in social science or closely related field or equivalent experience.
• 19 years or older
• Valid Driver's License
• Must pass a Background check prior to start date

  Monday - Friday
• Competitive Salary starting at $22.00 - $25.00 per hour or DOE
