If yes, here’s how you will contribute to the team:

As a QA Nurse (LPN/LVN), you will be responsible for monitoring, evaluating, and improving clinical systems within the Skilled Nursing Facility to ensure regulatory compliance, high-quality resident care, and accurate documentation.

Your key responsibilities include:

• Conduct routine audits of clinical documentation to ensure accuracy, completeness, and regulatory compliance.
• Review care plans to ensure alignment with physician orders and resident condition.
• Monitor Medicare/Medicaid documentation requirements and reimbursement-related compliance.
• Audit high-risk areas including falls, wounds, infections, weights, restraints, and psychotropic medications.
• Participate in and help lead QAPI meetings.
• Identify trends through data tracking and reporting.
• Develop and implement performance improvement plans.
• Monitor outcomes of corrective actions and process changes.

Must Have:

• Successfully graduated from an approved and certified LVN program.
• Current and unencumbered license to practice as an LVN in the state.
• Ability to make quick judgments and work independently.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.

Nice to Have:

• 2 years of experience as an LVN/LPN.
• Active patient or bedside care experience within the past three years.
