We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. This opportunity is open for a highly motivated and positive‑minded individual.
Your RoleThe LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks. The LVN is a licensed nurse who works in an Home Care Setting to provide nursing services.
Availability- Patients need care 7 days a week. Must be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).
Pay: $45.00 per visit

Experience- Visiting LVN for Home Health: 1 year (Required)
- Nursing: 1 year (Required)
- Current CA LVN License (Required)
- BLS from the American Heart Association or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Preferred)
Area or city close to you, with a cluster of patients who need care for that day.
Responsibilities- Providing nurse visits to home health patients (home visits).
- Care for ill, injured, or convalescing patients by implementing the plan of care initiated by an RN.
- Provide basic patient care and treatment. Document consistently with the plan of care.
- Communicate with patients to schedule visits.
- Educate the patient on how to take care during non‑visit days.
- Demonstrate knowledge of regulatory requirements.
