We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. This opportunity is open for a highly motivated and positive‑minded individual.

The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks. The LVN is a licensed nurse who works in an Home Care Setting to provide nursing services.

• Patients need care 7 days a week. Must be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

Pay: $45.00 per visit

Home Health

• Visiting LVN for Home Health: 1 year (Required)
• Nursing: 1 year (Required)

• Current CA LVN License (Required)
• BLS from the American Heart Association or the American Red Cross (Required)
• NSO Nursing License Insurance (Required)
• IV/Phlebotomy Certified (Preferred)

Area or city close to you, with a cluster of patients who need care for that day.

• Providing nurse visits to home health patients (home visits).
• Care for ill, injured, or convalescing patients by implementing the plan of care initiated by an RN.
• Provide basic patient care and treatment. Document consistently with the plan of care.
• Communicate with patients to schedule visits.
• Educate the patient on how to take care during non‑visit days.
• Demonstrate knowledge of regulatory requirements.
