Job DescriptionWe are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. This opportunity is open for a highly motivated and positive‑minded individual.
Your RoleThe RN travels to a patient's home to administer services and helps patients maintain their independence. Duties include administering at‑home IVs, changing dressings, cleaning wounds, performing in‑depth assessments, overseeing patient treatment plans, and updating doctors and the agency about patients’ health. Documentation and communication with the agency is done through Synergy Software.
AvailabilityPatients need care 7 days a week. You must be able to work at least two–three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).
Work AreaCity close to you, with a group of patients who need care that day.
Responsibilities- Providing nurse visits to home health patients (home visits)
- Performing an initial, comprehensive assessment and documenting everything in the Agency Portal
- Obtaining and documenting physician orders
- Initiating and regularly re‑evaluating the plan of care in consultation with the Clinical Team Manager
- Coordinating care with the Clinical Team Manager, physicians, and other home‑care team members, informing them of significant changes in patient condition and needs
- Performing re‑certifications, resumption of care, transfers, and discharges as requested, completing and submitting all related documentation
- Assessing the need for services of other team members (PT, OT, ST, MSW, HHA)
- Performing Home Health Aide supervisory visits per regulatory requirements
- Demonstrating knowledge of regulatory requirements
- Visiting RN for Home Health: 1 year (required)
- Nursing: 1 year (required)
- Current CA RN License (required)
- BLS from American Heart Association and/or American Red Cross (required)
- NSO Nursing License Insurance (required)
- IV/Phlebotomy Certified (required)
