We are seeking a dedicated and experienced Primary Care Physician to join our post-acute care practice near Hempstead, NY. • Patient Care:

  • Provide high-quality, safe, and cost-effective care to patients in post-acute care communities.
  • Manage a caseload of 15-20 patients per day, focusing on the unique needs of each individual.

• Collaboration:

  • Work closely with interdisciplinary teams, including hospice, palliative care, and home care agencies, to ensure safe transitions of care.
  • Collaborate with other clinicians to follow CMS guidelines and maintain high standards of care.

• Documentation and Billing:

  • Ensure timely documentation and billing through our EMR system (Gehrimed), meeting Medicare guidelines and ensuring compliance with all relevant regulations.

• Experience:
  • Willing to train new candidates; experienced candidates may be eligible for increased compensation due to prior experience in a post-acute care setting.
  • Board-certified or board-eligible in Family Medicine or a related specialty.
