Nurse Practitioner / Physician Assistant ? Skilled Nursing Facility

Do you want to make an Impact? Enjoy working with the Geriatric population? We are seeking to hire a Nurse Practitioner or Physician Assistant to join or team to provide medical care within a skilled nursing facility
.

No on-call requirement.

Responsibilities:

• Perform routine check-ups to patients to assess their health condition.
• Conduct examinations on acute changes in condition.
• Communicate with resident families regarding resident status.
• Manage and prescribe pharmaceuticals.
• Maintain accurate, comprehensive, and timely records on all encounters/interactions.
• Collaborate with providers, DON/ADON, nursing staff, support staff, and administration with a goal of improved patient care.
• Consult and utilize specialty services as required to provide comprehensive care to residents.

Requirements:

• Medical degree from accredited program.
• Active NY license to practice in field.
• Self-motivated.
• Strong understanding of examination methodologies and diagnostics.
• Excellent oral and written communication skills.
