Job Overview

The Health Coach is a non-clinical member of the Heart Failure care team, supporting patients through education, behavior change, and engagement within Novocardia’s value-based care model. This role partners closely with clinical team members to empower patients to better understand and manage their heart failure through prevention-focused, patient-centered coaching. The Health Coach plays a key role in improving patient experience, engagement, and self-management while maintaining clear boundaries from clinical decision‑making and care delivery.

Key Responsibilities

Patient Education & Engagement

  • Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management.
  • Establish short- and long‑term goals with patients using shared decision‑making and motivational interviewing techniques.
  • Encourage sustained patient engagement, self‑advocacy, and behavior change throughout program participation.

Care Team Collaboration

  • Partner with the clinical care team to support patient care plans and elevate concerns appropriately.
  • Provide timely updates and documentation related to patient engagement and progress.
  • Participate in interdisciplinary team meetings.

Patient Experience & Communication

  • Respond to patient communications in a timely, professional, and compassionate manner.
  • Build and maintain strong, trust‑based relationships with patients and caregivers.
  • Consistently deliver a high‑quality patient experience aligned with program standards.
Qualifications

Education & Experience

  • Bachelor’s degree in health sciences or related field preferred.
  • NBC‑HWC (National Board for Health & Wellness Coaching) certification required.
  • Experience in a healthcare, chronic disease management, or patient engagement setting preferred.
  • Cardiology or heart failure experience a plus.
  • Telehealth or remote patient engagement experience preferred.

Skills & Competencies

  • Strong communication and interpersonal skills.
  • Ability to motivate and support behavior change in diverse patient populations.
  • Organized, adaptable, and comfortable managing multiple priorities.
  • Proficiency with electronic health records and standard computer applications.
  • Bilingual English/Spanish proficiency preferred.
Work Environment
  • Remote position requiring a dedicated, private workspace and reliable high‑speed internet to ensure patient privacy and compliance with HIPAA standards.
