Overall Purpose
The Program Manager owns the day-to-day operation of P3's admission and medical expense reduction program across the Red Rock Region. Working alongside the VPMA, Provider Engagement Managers, Affiliate Practices, care management and other P3 Clinical Programs; this role identifies members with repeat inpatient admissions, assembles the clinical picture for weekly interdisciplinary review, translates that review into a clear plan, and delivers it directly to the primary care physician who can act on it.
The objective is straightforward: fewer avoidable admissions, earlier post-discharge follow-up, and better-supported patients at the most complex points in their care.
Work Location & Schedule
This role offers a hybrid work arrangement. Candidates will follow our hybrid schedule, working in office three days per week.
Essential Functions
- Own the weekly high-risk / frequent flyer interdisciplinary team (IDT) meeting end to end — build the agenda, prepare the patient list, capture decisions and track follow-through.
- Retrieve and synthesize clinical records across multiple affiliate electronic medical record systems to produce a concise case summary for every member under review.
- Translate each IDT decision into a one-page clinical impression and plan, and deliver it to the member's primary care physician.
- Contact each practice's designated point of contact directly by phone to review readmitted members and confirm next steps — clinic by clinic, not by electronic message alone.
- Conduct direct outreach to members in the cohort following an inpatient or emergency department admission, and connect them to a primary care appointment and to the P3 programs that fit their needs.
- Coordinate referrals to Council Care, P3 at Home, and hospice and palliative care, and track every referral from open to close.
- Support advance care planning and end-of-life conversations for members in the high-utilizer cohort, in partnership with the clinical team.
- Partner with data and analytics to maintain a current weekly roster of members with repeat admissions, refreshing the cohort as members enter and exit.
- Report program performance to market and Medical Affairs leadership against agreed measures, including admissions per 1,000, 30-day readmissions, post-discharge follow-up rates and referral completion.
- Build and document repeatable workflows so the program can extend to additional affiliate groups without rebuilding it each time.
- Protect patient privacy and maintain HIPAA compliance across all record handling, outreach and communication.
Knowledge, Skills, and Abilities
- Strong project management discipline — able to run a recurring clinical meeting, hold owners accountable and keep a moving patient list current.
- Comfortable reading and summarizing clinical documentation, including discharge summaries, problem lists and medication records.
- Skilled at building trust with physicians and practice staff, and at holding a peer-to-peer conversation about a shared patient.
- Clear, concise writing — able to reduce a complex case to a one-page summary a busy physician can act on in under two minutes.
- Working knowledge of value-based care, population health and utilization management in a managed care setting.
- Demonstrated ability to resolve complex problems effectively, with excellent judgment and decision-making skills.
- Proficient across multiple electronic medical record systems and standard collaboration tools.
- Sound judgment and discretion with sensitive clinical information, and consistent HIPAA compliance.
- Self-directed, organized and able to adapt in a dynamic and rapidly evolving healthcare environment.
