Hourly Salary Range: $43.00 - $45.00

• Complete an initial, comprehensive, and ongoing assessment of patients and families to determine hospice needs.
• Conduct physical assessments and document the history of current and previous illnesses.
• Provide professional nursing care by using all elements of the nursing process.
• Develop and initiate individualized care plans, including writing nursing diagnoses and setting goals.
• Regularly reevaluate patient status and revise care plans as needed.
• Record pain and symptom management data and document outcomes.
• Communicate with physicians regarding patient needs and report changes in condition; obtain and follow up on orders.
• Teach self‑care techniques to patients and caregivers.
• Collaborate with the interdisciplinary team to improve patient comfort and family support.
• Perform other duties as assigned.

• At least 1 year of acute‑care experience in an institutional setting.
• One year of experience in home care preferred.
• Current nursing license in the state.
• Understanding of hospice philosophy and the needs of terminally ill patients.
• Flexibility in work hours.
• Proficiency in applying competency‑based nursing skills.
• Willingness to maintain knowledge of information systems and related software.
• Ability to travel throughout the Bristol Hospice Service Area.
