Gentiva Hospice is seeking an After hours RN Care Manager to provide high‑quality hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends and on‑call hours.
You will assess needs, coordinate care, and maintain primary clinical oversight for assigned patients, while serving as an active IDG member to align physicians, families, and the hospice team.
