Role Overview :
The Home Health Physical Therapist provides patient-centered care in a home setting, focusing on recovery, mobility, and independence.
Responsibilities- Evaluate and assess patients' functional status, mobility, and strength
- Develop and implement personalized care plans in collaboration with the care team
- Administer hands-on therapy to improve mobility, balance, and strength
- Educate patients and caregivers on exercises, safety, and treatment plans
- Monitor progress, adjust treatments, and support safe care transitions
- Coordinate with nurses and physicians, document care accurately, and participate in team meetings
- Supervise PTAs and guide caregivers and support staff
- Graduate of an accredited PT program (DPT or Master’s)
- Current PT license in the practice state
- 1–2 years of experience, preferably in home health
- Strong clinical assessment, critical thinking, and communication skills
- CPR certification, valid driver’s license, reliable transportation
- Enjoys autonomy and field-based work
- Passionate about patient independence and functional outcomes
- Builds trusting relationships with patients and families
- Delivers clinical excellence with compassionate care
