Job Summary
As a Care Manager Registered Nurse, you will empower patient care through open collaboration with managers and participation in hospital councils and national nursing initiatives. You’ll help shape decisions that elevate patient outcomes and the future of nursing.
This position is eligible for a sign‑on bonus for qualified candidates.
Responsibilities- Provide case management services for both inpatient and observation patients as assigned.
- Identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another.
- Perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, along with an assessment of resources appropriate and available to the patient/family.
- Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary health‑care team, particularly with the patient’s physician, to facilitate a successful care transition.
- Partner with Social Services to ensure the post‑acute medical needs and level of care are appropriate.
- Evaluate progression of care using evidence‑based tools and approved criteria (Inter Qual) throughout the episode of care, escalating progression and transition of care issues through the established chain of command.
- Act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff, and outside agencies.
- Required Experience: 2+ years experience in Case Management OR 3+ years experience in a clinical setting
- Preferred Experience: Inter Qual experience
- Required
Education:
Associate’s Degree in Nursing - Preferred Education: Master’s Degree in Nursing
- Required License: Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
- Required Certification: BLS
