Registered Nurse - Home Health

Pay rate: $51/hr.

Setting: Home Health

Schedule:

40 hours weekly

Monday to Friday, 8 AM to 4:30 PM, with a rotating weekend schedule.

When scheduled for a weekend, the clinician will have Thursday off prior and Friday off the following week.

As a Home Health RN, you will provide personalized nursing care, helping patients recover from illness, manage chronic conditions, and maintain independence in the comfort of their own environment

• Provide skilled nursing care to patients in their homes, including private residences, assisted living, and adult foster homes
• Perform comprehensive patient assessments and develop individualized care plans
• Monitor patient progress and adjust care plans to support optimal outcomes
• Educate patients and families on disease management, medications, and recovery
• Collaborate with physicians and interdisciplinary teams to coordinate care
• Maintain accurate, timely documentation of assessments, treatments, and outcomes
• Advocate for patients and support their safety, comfort, and well-being

• At least 1-year of recent experience in acute care, critical care, emergency department, Community Health, Home health, or a similar setting is required.
• Active Basic Life Support (BLS) or be willing to obtain it post-offer
