Location: Hermiston, OR
FLSA: 1.0 FTE Non-Exempt (40 hours per week, Mon-Fri 8:00am-5:00pm)

Job Title: ACT Nurse/Nurse
Pay Grade: N14 ($53.94 - $65.29, depending on experience)
Supervisory/Bonuses: Potential for 10% bilingual increase (must pass language test after hire); $5,500 Hiring Bonus (2 year commitment, staggered-taxed payout)

The ACT nurse, supervised by the ACT Team Leader/Clinical Supervisor, provides culturally competent mental health nursing care to participants in the ACT program. The nurse delivers treatment in accordance with Oregon Administrative Rules (OARs) and CCS mission/values. The nurse works with the multidisciplinary ACT team to provide recovery, treatment, and support to clients, including medication oversight and coordination with community partners.
Responsibilities- Administers oral medications and injections as prescribed; records vital signs and patient condition information
- Lead prescribed individual, group therapy and/or education sessions
- Coordinate with community agencies and families to maintain treatment continuity
- Assist participants in taking medications independently using coaching and reminders
- Dispense medications from dispensary and maintain records per procedures
- Educate and train staff on medication administration; ensure documentation according to policies
- Participate in joint case planning; liaise with clients and other agencies; advocate for clients when appropriate
- Maintain client records using electronic medical records; document interactions, assessments and service plans per program standards
- Triages and assesses medical concerns; make referrals as needed
- Formulate nursing care plans and participate in care meetings
- Consult with health team members as needed
- Complete all paperwork professionally and timely per OARs and guidelines
- Other duties as assigned
- Current Registered Nurse license issued by the state of Oregon
- Criminal history background check clearance
- Education/
Experience:
sufficient education to qualify as a registered nurse; related experience helpful - Preference given to those with mental health experience in residential or psychiatric settings
- Knowledge of mental illness and addictions, and relevant OARs
- Strong verbal and written communication; ability to work independently and coordinate across agencies
- Ability to develop cooperative relationships with clients and families; understanding of the recovery model
- Ability to represent the agency positively in the community; teamwork skills
- Respect client confidentiality; handle sensitive information
- Proficiency with medical terminology and business English; strong organizational and time-management skills
- Ability to use standard office equipment
- Pre-employment drug screen
- Must hold a valid driver’s license and be insurable for CCS-related travel
- Pass criminal history checks as required by ORS/OARs
Regularly required to use hands; stand, walk, stoop, kneel, crouch; occasional climbing or balancing. Lift up to 10 pounds. Vision requirements as needed. Work performed in secure facility; potential community travel and occasional overnight stays. Reasonable accommodations available.
