Good Shepherd Health Care System is seeking an experienced Registered Nurse (RN) Case Manager for a 13-week contract opportunity in Hermiston, Oregon.

The RN Case Manager is responsible for assessing, planning, coordinating, implementing, and monitoring comprehensive patient care plans while collaborating with interdisciplinary teams, patients, families, community resources, and post-acute care providers.

• Perform comprehensive patient assessments including clinical, psychosocial, environmental, and financial needs
• Develop and manage individualized care plans
• Coordinate patient transitions between care settings
• Facilitate discharge planning and continuity of care
• Collaborate with physicians, nurses, therapists, social workers, and community agencies
• Monitor length of stay and identify discharge barriers
• Obtain insurance authorizations and evaluate medical necessity
• Educate patients and families regarding diagnoses, treatments, and self-care
• Connect patients with community resources and support services
• Maintain accurate case management documentation and records
• Participate in interdisciplinary care conferences

• Minimum 3+ Years of RN Experience
• Previous Acute Care or Home Health Experience Required
• CRASE Training Required
• BSN/MSN Preferred
• Strong communication, documentation, and care coordination skills

• Experience working with culturally diverse populations
