DescriptionAmerican Traveler is hiring an RN Case Manager for a Home Health setting requiring an active Oregon RN license and prior acute care or home health case management experience.
Details- Home Health setting
- Day shift schedule, 5 shifts per week at 8 hours per shift
- Uses Epic EMR
- Agency nurses may be floated to other departments before core staff are floated
- BLS certification preferred
- CRASE training required; if assignment extends beyond 6 months, CRASE training will be required
- Minimum two years of experience required
- Previous clinical experience in acute care or home health assessing skilled care needs and resource requirements required
- Case Management Certification preferred
- BSN/MSN preferred
- Assess patients' healthcare needs and develop comprehensive care plans
- Coordinate transitions between care settings and ensure continuity of care
- Evaluate medical necessity for treatments and collaborate with insurance companies for authorizations
- Monitor length of stay and identify discharge barriers
- Advocate for patients' needs and connect them with community resources
- Educate patients and families on diagnoses, treatments, and self-care
- Maintain accurate case management documentation and track quality metrics
- Collaborate with an interdisciplinary team including physicians, therapists, and social workers
- Work with a culturally diverse patient population across all age groups
- Support the organization's mission, vision, values, and required education programs
