Case Manager

The
Case Manager
at Palmer Home for Childrenis responsible for supporting the holistic well-being of each childincareby coordinating individualized care plans, ensuring their physical, emotional, educational, and spiritual needs are met. This role works closely with caregivers, therapists, teachers, and other team members tomonitorprogress,facilitateservices, and advocate for the child’s best interests, while upholding Palmer Home’s mission and trauma-informed care approach.

• Develop andmaintainpositive working relationships withchildrenand those engaged in the life of the child.
• Act as a liaison between Palmer Home and families
• Make regular visits to children and youth incareandhomes of birthfamiliesorguardians toassess needsin accordance with Palmer Home processes and procedures.
• Provide ongoing communication with birth familiesor guardiansof placed children, working towards reunification as the primary goal.
• Completeassessmentsofchildren and families
• Develop and effectively communicatedocumentedcase plans, recommendations, visits,and contact with birth families,guardians,children,caregivers, service providers, and agencies.
• Maintain appropriate documentationset forth by Palmer Home Social Services.
• Create and managefilesfor placed children in accordance withagencyrequirements
• Review and manage case strategies/whole child reviews for each child to ensure that theobjectivesof each child’sindividualizedcase plan are achieved, regularly communicating those results to the Director of Children’s Programs.
• Contribute to the development ofongoing training and support for families
• Participate in ongoing employee training as required.
• Act asa resourceforfamilyin connection with the schools and counselors.
• Act asa resource to help with medical appointments, scheduling, and records as needed.
• Connectchildren, families, and caregivers to appropriate community resources.
• Assistin scheduling and supervision of familyand siblingvisits.
• Work withmentorfamilies to ensure that theymaintaina safe and consistent environment for the children visiting their homes.
• Facilitate regular caregiver meetings andchild(ren) check-ins.
• Be available for emergencies as needed during work hours as well as after hoursand weekends on a rotating schedule as needed.
• Supply notary services as needed.
• Assistin theworkof reunification plans for children and families which may include but is not limited to travel, home visits, emergencies, homesafety checks.
• Complete Consent/Release forms for children and families as needed for events, marketing,etc.
• Work fully within the philosophy, goals and operating procedures of Palmer Home for Childrendemonstratingexemplary Christian character.

• Bachelor'sdegreein social work andminimumof three years’ experience incasework capacity.
• Have an active Social Work licenseor willing to obtain a Social Work license subject to thetime frameset forth by Palmer Home(LSW/LMSW/LCSW)
• Knowledgeofand willingness to work alongsidechildren, families, caregivers,local social servicesagenciesand programs with which Palmer Home mustfrequentlyinteracttoprovideadditionalservices to our children.
• Strong interpersonal skills.
• Strong team-work skills.
• Possess necessary skills to effectively administerprograms,communications, andhuman relations.
• Notary Certified or willing to become annotary w/in 6 months of hire.
• Willing to travel as requiredor needed to successfullyaccomplishrequirements of this position
• Mustbe in agreementwith the Palmer Home for Children Statement of Christian Beliefs anddemonstratea strong commitment to Christ consistent with the mission of Palmer Home status as a non-profit religious organization.
