** Position Summary*
* • Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.
** Education*
* • Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience.
** Licenses and Certification*
* • Illinois license in Physical Therapy  
• BLS
** Experience and Skills*
* • Technical

• Develops appropriate Plan of Care  
• Implements Plan of Care  
• Documentation is accurate, complete and timely
