Job Description & Requirements

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to provide patient care in Hershey, Pennsylvania.

Responsibilities
  • Lead and collaborate to manage and deliver patient care plans.
  • Serve patients in Hummelstown, PA, and surrounding areas.
Qualifications
  • RN license in the state where you work.
  • Graduate from an approved school of practical nursing.
  • One year of RN experience and the clinical competence to deliver quality patient care.
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
  • Commitment to consistently meet critical deadlines for charting.
  • Ability to self‑manage time and schedule.
  • Demonstrated experience with tablets, mobile phones and EMR software.
Compensation Highlights
