Job Summary

Penn State Health has an opening for a Nurse Practitioner for Post Acute Care. As part of a multidisciplinary team, patient care will be provided at Long-term care facilities and Rehab Facilities, in conjunction with a Physician Board certified in both Internal Medicine and Geriatric Medicine. This position will include Internal Medicine consultation services for patients in Long Term Care and Rehab Facilities.

The NP functions as a member of a multidisciplinary health care team and provides a wide range of patient services to a specific patient population across the continuum of health care. By utilizing the nursing process, scientific reasoning and critical problem solving, NPs practice within the scope of responsibilities and prescriptive authority defined by the Pennsylvania State Board of Nursing and the Medical Staff Executive at the Penn State Hershey Medical Center.

Minimum Qualifications
  • Master's Degree in Nursing with clinical specialization with a minimum of two years full-time work experience as a RN
  • Minimum of 1 year experience as a NP, which may include a clinical student practicum
  • Current active licensure in the state of Pennsylvania
  • Demonstrated effective skills in consultation
Preferred Qualifications
  • Previous Internal Medicine Experience. Inpatient and/or LTAC-Rehab clinical experience. Experience working with the Geriatric population
