The Licensed Practical Nurse (LPN) will provide direct and individualized nursing care to an assigned group of patients under the direction of a registered nurse or provider. Responsibilities include observing, reporting, and recording medical information, documenting care in the electronic medical record, instructing patients and families, and communicating with the healthcare team to coordinate individualized patient care.

• Provide direct and individualized nursing care to an assigned group of patients under a registered nurse or physician, in compliance with the Pennsylvania Nurse Practice Act and institutional policies.
• Observe, report, and record medical information and nursing care in the medical record or electronic medical record.
• Administer medications in accordance with policy and procedures and instruct patients/families per the teaching plan overseen by a Registered Nurse/Physician.
• Communicate with the healthcare team to facilitate the coordination of individualized patient care.
• Assist in the timely management of phone messages, e‑messages, and form completion, in accordance with organizational policy.
• Maintain professional knowledge and skills by attending in‑service training, continuing education, and other learning opportunities.
• Participate in problem‑solving, planning, and improvement of patient care activities, including research, quality improvement, and evidence‑based practice initiatives.

• Currently licensed as a Practical Nurse in the state of employment or holding a multistate LPN license through the Interstate Nurse Licensure Compact.
• Basic Life Support (BLS) certification required prior to completion of orientation.
