Are you an experienced Registered Nurse (RN) leader in case management, care coordination, or utilization management ready to drive system-level impact in an academic medical center? We are seeking a Director of Case Management to provide strategic and operational leadership across case management, utilization review, pediatric case management, care coordination, discharge planning, and clinical social work.

Your Impact as a Nursing Leader

Lead a team of 5 managers and multidisciplinary clinical teams including RNs, social workers, and care coordinators delivering patient-centered care. Drive excellence in clinical case management, improve care transitions, and advance population health and value-based care initiatives.

Key Responsibilities Strategic Leadership in Case Management & Nursing Operations

Lead strategy for case management, care coordination, and utilization review while ensuring alignment with CMS, Joint Commission, and payer requirements.

Operational Excellence in Care Coordination

Oversee utilization management, discharge planning, and transitions of care. Optimize length of stay, patient throughput, and readmission reduction.

Mentor and develop leaders, RN case managers, and social work teams. Establish staffing models and support professional development.

Analyze data including LOS, readmissions, and denials. Lead performance improvement and partner with finance and revenue cycle.

Academic & Interdisciplinary Collaboration

Support education, research, and interdisciplinary collaboration in a teaching hospital environment.

Qualifications (Nursing & Case Management Leadership)

Registered Nurse (RN) required. Leadership experience in case management, care coordination, or utilization management. Knowledge of regulatory standards and value-based care.

MINIMUM QUALIFICATION(S)
  • Bachelor's Degree in Nursing
  • Minimum five (5) years management experience
  • Minimum seven (7) years related functional area expertise experience
PREFERRED QUALIFICATION(S)
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM)
