Care Manager RN - Float Personnel

The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures and documented workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model.

• Graduate from an accredited school of Nursing.
• California Registered Nurse License (upon hire).
• 2 years Professional Nursing Experience.

• Bachelor's degree in nursing.
• 1 year of experience in ambulatory care nursing.

The wage range for this position is $57.28 to $88.92. Placement on the range will be based on relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, and bonus opportunities may be available.
