Social Worker, Patient​/Health Advocate

Catholic Health Services seeks a compassionate Social Worker to support patients and families across our South Florida healthcare network. In this role, you will complete psychosocial assessments, develop care plans, and coordinate services across hospitals, elderly care, and home health settings. You will provide counseling, crisis intervention, and discharge planning while linking patients to community resources. Collaborating with an interdisciplinary team, you will advocate for patient needs, maintain accurate documentation, and uphold CHS's mission of respect, dignity, and holistic, patient-centered care.
