Overview
The APRN is a clinical role that will provide exceptional medical care to patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under physician supervision. This safety‑sensitive position involves direct patient care and medication administration. The incumbent reports to the Regional Manager and works closely with other healthcare professionals to deliver quality care.
Responsibilities- Practice medicine under physician supervision with sound medical judgment and ethical practice.
- Provide care for acute and chronic illnesses in older adults.
- Accountable for patient care and outcomes, seeking specialty consultation as needed.
- Engage with hospitalists for hospitalized patients.
- Meet all clinical obligations (accreditation agency compliance, CLIA, drug prescription compliance, HIPAA, State and Federal regulations, safety regulations).
- Assess, diagnose, treat, manage, educate, promote health, and provide care coordination using evidence‑based medicine.
- Follow internal policies and procedures.
- Complete operational tasks per departmental guidelines.
- Maintain detailed EMR records within 48 hours of each visit.
- Provide patients with health promotion information.
- Analyze and interpret patient histories, symptoms, physical findings, and diagnostic data.
- Interpret diagnostic results (blood gases, radiology, EKG, etc.) with supervising physician.
- Demonstrate compassion, quality, loyalty, and conscientiousness.
- Establish effective working relationships with patients, families, and team members.
- Document continuing education per licensure and certification requirements.
- Ensure patients are seen every other visit by the Physician.
- Maintain patient confidentiality per policy and HIPAA.
- Enact proper chain of command for patient complaints.
- Perform additional duties as assigned.
- Graduate of an accredited school of nursing.
- Certified nurse practitioner with current state licensure.
- At least 1 year of nursing experience; 1–3 years in primary care NP role.
- Experience with electronic medical records systems.
- Provide five‑star service aligned with Better Health Group’s mission.
- Skilled in patient assessment and management.
- ACLS/BLS certification required.
- Knowledge of appropriate patient care rationales.
- Calm and effective emergency response skills.
- Analytical skills for interpreting business and regulatory information.
- Effective presentation and communication skills.
- Strong written and verbal communication, teaching ability, telephone skills.
- Proficiency with Google Suite and Microsoft Office.
- Full range of body motion; handle and transfer patients, dexterity, eye‑hand coordination.
- Stand, walk, push, bend, kneel, and reach continuously in a clinic setting.
- Lift and move patients and medical equipment; lift/carry up to 15 lbs.
- Corrected vision and hearing within normal range.
- Sit for extended periods.
- Drive a motor vehicle and have own transportation.
