Job Description:Shift: Monday - Friday, 8-4:30
Location: Advent Health Winter Garden
- Acts as a liaison across the multidisciplinary team to ensure proper orders, medical records, consents, and other required documentation are received.
- Coordinates timely care with physicians and supportive services departments.
- Maintains professional relationships with customers, patients, and team members.
- Provides direct patient care coordination within the Community Care and Transitional Care Programs.
- Educates patients, families, caregivers, and community providers to safely perform care and achieve health goals.
- Evaluates the effectiveness of care plans and monitors patient and family responses to achieve desired outcomes.
- Identifies performance improvement initiatives and assists in designing or implementing programs to address needed changes.
- Participates in community education and outreach programs to promote health and wellness. Collaborates with Imaging Coordinator, RN Navigator, and referring providers to navigate patients through treatment planning, medical management, and surveillance.
- Serves as an educational resource for patients and families on treatment planning and medical management.
- Facilitates communication between healthcare team, patients, other departments, and outside entities by answering phone calls, responding to emails, and managing messages in the EMR and patient portal.
- Ability to provide age-appropriate cancer education. [Required]
- Strong organizational, communication, and customer service skills. [Required]
- Bachelor's [Preferred]
- Graduate of an accredited school of nursing
- in nursing
- 1+ professional nursing experience [Required]
- 1+ professional nursing experience in oncology [Preferred]
Certifications:
- Registered Nurse (RN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:$68,132.50 - $
