Big Bend Hospice is seeking a Homecare RN that provides comprehensive case management services for hospice patients across care settings. This role combines the critical responsibilities of patient assessment, care coordination, and interdisciplinary group (IDG) collaboration to ensure seamless, high-quality hospice care from initial referral through ongoing support.

The Homecare RN serves as a clinical leader who bridges intake and ongoing care, maintaining regulatory compliance, promoting the hospice philosophy, and ensuring excellence in patient and family experiences.

Duties/Responsibilities
  • Evaluates patients and families for admission to BBH using approved criteria and procedures.
  • Conducts pre-assessment and admission visits in the home, hospital, or facility setting.
  • Coordinates with referring physicians, intake teams, and community partners to promote timely and appropriate hospice admissions.
  • Provides education to patients, families, and referral sources on hospice philosophy, eligibility, and available services.
  • Ensures all admission documentation is accurate, complete, and submitted within agency timeliness standards (documentation due by midnight each day).
  • Develops, coordinates, and implements the comprehensive IDG plan of care to meet the individualized needs of patients and families.
  • Provides skilled nursing care, symptom management, and patient/family education consistent with hospice best practices.
  • Maintains regular communication with physicians, IDG members, and community partners to ensure continuity of care.
  • Supervises and guides Licensed Practical Nurses (LPNs) and Hospice Aides (HAs), initiating their respective plans of care.
  • Participates in interdisciplinary team meetings, quality assurance activities, and continuous improvement efforts.
  • Demonstrates knowledge and competency in pain and symptom management, end‑of‑life support, and holistic care practices.
  • Ensures accurate, timely, and compliant documentation in the electronic medical record (EMR).
  • Demonstrates thorough understanding of Medicare, Medicaid, and regulatory requirements governing hospice care.
  • Adheres to all BBH, State, and Federal policies, procedures, and laws relevant to hospice practice.
  • Serves as a clinical resource and mentor to peers, new hires, and students.
  • Represents Big Bend Hospice with professionalism and compassion to patients, families, referral sources, and community partners.
  • Works collaboratively with the Admissions, Education, and Community Relations teams to ensure smooth transitions from referral to ongoing care.
  • Participates in weekend, evening, and holiday rotations as assigned.
  • Maintains a balanced caseload in alignment with organizational standards.
  • Demonstrates accountability, sound clinical judgment, and effective prioritization in managing patient care.
  • Practices fiscally responsible use of resources and supplies.
  • Performs other duties as assigned.

  • Must possess a valid Florida driver's License, valid auto insurance, and reliable transportation.
Education and Experience

Minimum of one (2) year of inpatient nursing experience or previous hospice/home health and/or Long‑term Care experience preferred. Must maintain a current valid Florida Registered Nursing License.
