Hospice LPN Continuous Care; PRN

Overview

This position is based in Hialeah, FL and requires daily travel within an assigned territory in Miami Dade County.

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g., foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.

• Bilingual in English/Spanish REQUIRED
• One year of clinical LPN experience REQUIRED
• Current LPN license, specific to the state(s) you are assigned to work
• Current CPR certification
• Valid driver's license, reliable transportation, and liability insurance

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities and patients’ homes
• Proficient in electronic medical record (EMR) systems
