About the Role:

We are seeking highly skilled and compassionate Nurses to join our team of healthcare professionals. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care to our residents, working collaboratively with our multidisciplinary team of healthcare professionals, and contributing to a positive and supportive work environment.

Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and other treatments as needed

• Conduct initial and ongoing assessments of patients to identify and address their physical, emotional, and social needs

• Develop and implement individualized care plans to meet the unique needs of each patient

• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care

• Monitor and report patient progress, identify potential problems, and develop strategies to address them

• Provide education and support to patients and their families to promote independence and self-care

• Participate in quality improvement initiatives to ensure the highest level of care is provided to our patients
