Catholic Health Services seeks a compassionate Social Worker to support patients and families across our South Florida healthcare network. In this role, you will complete psychosocial assessments, develop care plans, and coordinate services across hospitals, elderly care, and home health settings. You will provide counseling, crisis intervention, and discharge planning while linking patients to community resources. Collaborating with an interdisciplinary team, you will advocate for patient needs, maintain accurate documentation, and uphold CHS's mission of respect, dignity, and holistic, patient-centered care.
Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Develop and implement individualized care and discharge plans
- Provide counseling, crisis intervention, and emotional support
- Coordinate community resources, benefits, and support services
- Collaborate with physicians, nurses, therapists, and care teams
- Advocate for patient rights and access to services
- Document assessments, interventions, and outcomes in EHRParticipate in case conferences and care transition planning
- Educate patients and families on care options and resources
- Support CHS's mission of compassionate, faith-informed care
