Position Overview

As the MDS Coordinator, you will oversee the full RAI process, ensuring timely and accurate completion of MDS assessments while supporting care planning and reimbursement accuracy.

This role is ideal for someone who thrives on organization, precision, and driving clinical excellence.

Key Responsibilities
  • Ensure timely completion of all MDS assessments
  • Maintain accuracy of documentation and regulatory compliance
  • Manage MDS transmissions and validate state submissions
  • Coordinate and lead interdisciplinary care plan conferences
  • Monitor and improve Quality Measures (QM)
  • Collaborate with nursing, therapy, and leadership teams
  • Ensure care plans reflect resident needs and regulatory standards
Qualifications
  • Active, unencumbered RN or LPN license
  • Minimum of 2 years of MDS experience in long-term care
  • Strong knowledge of the RAI process and care planning
  • RAC-CT certification preferred
