Overview

Private Duty Nurse (PDN)-LPN is a member of the client care team that provides services directly, or acts as a placement agency, to deliver skilled nursing services to clients in their personal residences. The PDN LPN works under the supervision of the Clinical Manager.

Responsibilities
  • Evaluates and treats clients with appropriate level of skill according to client’s needs under the supervision of an RN, Clinical Manager.
  • Assist with providing continuity of care and emotional needs of client and family.
  • Resourceful in meeting client’s psychosocial needs.
  • Verbal demonstration of client rights and responsibilities.
  • Make decisions based on sound nursing judgements.
  • Recognizes and copes with emergencies appropriately.
  • Ensures proper management of client services.
  • Demonstrates competent use of equipment.
  • Adequately communicates information relative to client care in accordance with agency policies.
  • Observes the client’s general physical, emotional, and mental status, and reports all changes in the client’s conditions, family situation, incident, and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator.
  • Ensures all forms of communication with clients, families, and health plan providers are documented timely to promote continuity of care across all interdepartmental functions.
  • Builds relationships with clients to encourage compliance with care plans and client safety.
  • Maintains accurate clinical and shift work documentation to update system in accordance with agency guidelines in a comprehensive and timely manner.
  • Manages client/contract satisfaction with the highest level of customer service standards, communication, and documentation.
  • Adheres to infection control policies and exposure control plan as mandated.
  • Monitors and maintains clinical reports, including Infection and Occurrence/Complaint Reports.
  • Adheres to and promotes safety and injury prevention measures in client and workplace settings.
  • Maintains and improves skills through attendance at staff development programs and in-service education requirements.
  • Participates in community outreach activities that promote the goals and objectives of the agency.
  • Maintains confidentiality in all professional areas and considers the needs of clients and their families, other employees, physicians, visitors, and all members of the community in an informative, courteous, and compassionate manner.
  • Ensures all HIPAA regulations are adhered to.
  • Follow Agency Policies and Procedures.
  • All other duties as assigned.
ADA Requirements
  • Routinely required to sit, stand, and walk on variable surfaces, including occasional climbing of ramps or stairs or the use of footholds.
  • Routinely required to use hands for various tasks, including hand controls, gripping, and handling of objects.
  • At times, will be required to reach beyond mid-line of the body, reach overhead or at chest level.
  • There are specific vision abilities required to perform this job, which include close, distance and peripheral vision, depth perception and ability to adjust focus.
  • All employees will be required to use universal safety precautions when performing their assigned tasks.
  • Employees must have the ability to lift, push, pull and carry a minimum of 25 pounds.
  • Employees must have the ability to stoop and bend up to 2.5 hours per 8-hour shift.
Requirements Qualifications
  • Education:

    Graduate of an accredited school of nursing.
  • Licensure / Certification:
    Licensed practical nurse (LPN) who is licensed by the North Carolina Board of Nursing (NCBON). Must have a current, unencumbered license as an LPN, or an Advanced Practice Nurse. Must be in good standing in the applicable state of practice. Must maintain all state and federal compliance requirements for licensure and employment.
  • Travel:
    Travel is required for business purposes. Employees must have a valid driver’s license issued by the state in which they work and a satisfactory driving record.
  • Work Experience:

    Minimum of one year of home care experience. One additional year of nursing experience, preferably in a hospital, is required for conducting applicable training classes.
  • Other Hours of Work:
    Varies on a case-by-case basis. Full time is at least 30 hours a week from Saturday-Friday.
  • Skills and

    Additional Requirements:
  • A specific understanding of geographical/cultural requirements of branch office is preferred.
  • Must be customer service oriented and prepared to liaise with patients, their families and other care providers.
  • Ability to use computerized systems.
  • Satisfactory verbal and written communication skills.
  • Ability to work well under pressure.
  • Bi-lingual preferred
