Licensed Practical Nurse – Home HealthAs a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence – caring for patients like family, one home visit at a time.
Responsibilities- Provide nursing care in the home, including wound care, medication administration, and support during procedures.
- Document assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care.
- Monitor patient status and alert the care team of any changes to ensure timely, coordinated responses.
- Educate patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home.
- Collaborate closely with RNs, providers, therapists, and care coordinators to deliver high‑quality, personalized care in the home setting.
- Must obtain Basic Cardiac Life Support certification within 15 days of employment start date.
- Valid Driver’s License (DL).
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
- Associate Degree in Nursing or graduation from a Vocational School.
- Minimum 1 year of nursing experience required.
