Brief Description
Authora Care Collective is currently seeking a Medical Assistant Care Coordinator. This position is Full‑time:
Monday‑Friday 8:00 am‑5:00 pm, office‑based in High Point. The role supports patients receiving home‑based primary care, palliative care, chronic care management (CCM), and other care models through face‑to‑face visits, virtual visits, telephonic assessments, and phlebotomy services. The coordinator verifies continued clinical need, schedules timely follow‑ups, and addresses the physical, psychological, emotional, and spiritual needs of individuals with life‑limiting or chronic conditions.
Must be a graduate of a Medical Assistant Program (CMA preferred). The coordinator contributes to patient care through assessment, intervention, care planning, care coordination, and documentation, plays a key role in care transitions, program development, efficiency in front‑office workflows, and communication across healthcare teams. RequirementsEducation and Experience:
- Graduate of a Medical Assistant Program (CMA preferred).
- Certified or trained in phlebotomy.
- Minimum of 1 year of clinical experience; 2 years preferred in post‑acute, hospice, long‑term care, or home health.
- Experience with Chronic Care Management or Population Health, or Primary Care preferred.
- Proficiency with Microsoft Word, Excel, and Athena strongly preferred.
- Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by agency and have own transportation.
