Position Summary
Authora Care Collective is currently seeking a Physical Therapist (PT) Case Manager for our Home Health department supporting our High Point, Kernersville, and Winston‑Salem service areas. This position is Full‑Time:
Monday – Friday 8:00 am – 5:00 pm.
The Physical Therapist provides services to restore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease. Manage the therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions. Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands‑on therapy, and equipment to ease pain and help increase mobility and endurance.
Key ResponsibilitiesEstablish and maintain therapeutic relationship with patient, including providing community support for resources and education. Document the goals and outcomes of care. Therapist may perform evaluations and treatments for hospice patients as needed.

Requirements- Graduate degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one‑year physical therapy experience (home health or facility preferred)
- Current and unrestricted Physical Therapy Licensure
- Current CPR certification
- Patient management skills needed include excellent patient assessment, critical thinking, problem solving, delegation, effective interpersonal and communication skills.
Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
