Location:
Hospice of the Piedmont, 1801 Westchester Dr., High Point, NC 27262, USA. This is a full-time, salaried, exempt position.
Schedule:
Friday, Saturday, Sunday 8am-8pm (36 hours). Location covers both Asheboro, NC and High Point, NC with daily travel required.
- Collaborate with the Medical Director, Hospice Providers, and Community Primary Care Providers to deliver hospice and palliative care services.
- Round at inpatient units and other settings as assigned; serve as the hospice patient’s attending medical provider while in the inpatient unit.
- Develop and implement a medical plan of care in accordance with patient wishes and in full compliance with regulatory requirements including the Nurse Practice Act and agency policies, procedures, and standards of care.
- Assist with program development, provide education in hospice and palliative care, and serve as a resource to the interdisciplinary team.
- Provide medical staff on-call coverage for hospice and palliative care as assigned.
- Provide consultation with inpatient and outpatient referrals, review medical charts, communicate with health care staff, visit patients and caregivers, and perform comprehensive assessments to identify symptom issues and goals of care.
- Establish therapeutic relationships with patients and caregivers, provide emotional support, and guide patients and families through disease progression and bereavement considerations.
- Document in patient medical records in accordance with federal and state regulations, standards of practice, and organizational policy; collaborate with physicians and other health care providers.
- May serve as the hospice patient’s attending physician if the patient does not have a medical provider and requests the NP as attending physician.
- May perform home visits and, when appropriate, issue medical orders in coordination with the patient’s attending physician.
- Refer to other health care providers as indicated by patient health care needs.
- Graduate of an accredited school of nursing.
- Current North Carolina Registered Nurse and Nurse Practitioner licenses.
- Hospice and palliative care experience and certification preferred.
- Valid NC driver’s license, reliable independent transportation, and current automobile liability insurance at agency-required limits.
- Ability to collaborate with agency staff and volunteers as part of the interdisciplinary team.
- Ability to maintain positive working relationships and courteous interaction with the public.
- Excellent verbal and written communication.
Travel Required:
Yes. Daily travel required.
