Description

Live Long Well Care is seeking an experienced Home Care RN. Office is conveniently located just off of I‑40 and Stratford Road. The position is responsible for providing direct care and compliance support for nursing, and may assist with outreach to prospects, hospitals, physician offices, rehab clinics, and senior organizations. This private‑pay (non‑Medicare/Medicaid certified) role provides private duty skilled and non‑skilled services for the senior/elderly population.

Reports to the CSM (Client Services Manager).

Position Summary

Provides hands‑on resident care, proper and timely administration of medications and treatments, accurate verbal and written reports, and rapid response to all emergency situations. Level I – Basic competencies and knowledge of nursing:
Client Examination and Assessments.

Essential Functions
  • Performs professional and restorative nursing services: resident assessments, recording of vital signs, administering medications and treatments.
  • Maintains timely documentation of written and verbal reports ensuring quality care.
  • Interacts with the resident’s physician, pharmacist and medical personnel as needed.
  • Follows guidelines in the Resident Care Plan; assists residents with activities of daily living including bathing, dressing, eating, toileting, hygiene, and mobility.
  • Assists with in‑service and on‑the‑job training and mentoring of team members.
  • Participates in and attends all required in‑service training and education programs as scheduled.
  • Other duties as assigned.

Physical Demands, Work Environment, and Expected Hours of Work

The position is very active and requires frequent standing, walking, bending, kneeling, stooping, and climbing. Lifting or carrying up to 50 lbs and pushing up to 250 lbs independently may be required. Proper body mechanics must be used to assist residents in daily living. The role involves long hours and frequent night and weekend work.

Travel

No regular travel is expected; occasional travel may be required for training sessions, continuing education, emergency situations, and other company functions.

Education and Experience Requirements
  • Current state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse in good standing.
  • Certification in CPR, AED, and First Aid.
  • High school diploma or GED.
  • Four‑year college or university degree desired.
  • 1‑2 years’ experience in a similar healthcare position preferred.
Knowledge, Skills, and Abilities Required
  • Ability to read, write, speak, and understand English fluently.
  • Ability to meet or exceed the company’s attendance and punctuality standards.
  • Ability to use Electronic Records and miscellaneous software and office equipment.
  • Ability to understand and follow directions as given.
  • Ability to work with minimal supervision.
