Job Title:
Licensed Practical Nurse (LPN) – Home Health Position Summary
The Licensed Practical Nurse (LPN) provides direct, hands‑on nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient independence, safety, and comfort while following individualized care plans and agency policies.
Essential Duties and Responsibilities- Provide skilled nursing care to patients in their homes according to the plan of care
- Monitor and document vital signs, symptoms, and overall patient condition
- Administer medications and treatments as prescribed
- Perform wound care, catheter care, ostomy care, and dressing changes
- Assist patients with activities of daily living (ADLs) as needed
- Observe and report changes in patient condition to the supervising RN or physician
- Provide education to patients and caregivers on disease management, medication use, and safety
- Maintain accurate, timely clinical documentation in accordance with regulatory requirements
- Follow infection control, safety, and confidentiality standards
- Collaborate with interdisciplinary team members to ensure coordinated care
- Participate in case conferences, in‑service training, and quality improvement activities
Education and Licensure
- Graduate of an approved Practical Nursing program
- Current LPN license in the state of practice
- CPR certification (BLS)
Experience
- Previous home health or clinical nursing experience preferred
- Experience with geriatric or chronic care patients a plus
- Knowledge of basic nursing principles and home health practices
- Ability to work independently and make appropriate clinical decisions
- Strong assessment, communication, and documentation skills
- Compassionate, patient‑centered approach to care
- Ability to follow care plans and physician orders accurately
- Basic computer skills for electronic health records
- Reliable transportation and ability to travel to patient homes
- Work performed in patients’ homes and community settings
- May require travel, flexible scheduling, evenings, weekends, or holidays
- Ability to lift, reposition, and assist patients as needed
