Job SummaryTRITAL Care is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for an opportunity in High Point, North Carolina. This role is ideal for an experienced LPN who is confident in patient care, medication administration, documentation, and supporting residents or patients in a professional clinical setting. The selected nurse will work closely with RNs, physicians, CNAs, and facility leadership to deliver safe, respectful, and high-quality patient care.
Key Responsibilities- Providing direct nursing care to patients or residents according to the care plan.
- Administering medications and treatments as ordered.
- Monitoring patient condition and reporting changes to the RN, provider, or charge nurse.
- Assisting with admissions, discharges, transfers, and routine clinical documentation.
- Supporting wound care, vital signs, specimen collection, and basic bedside procedures as permitted by scope.
- Maintaining accurate charting in the facility's EMR system.
- Following infection control, safety, HIPAA, and facility compliance policies.
- Supervising or supporting CNAs and nursing assistants when assigned.
- Delivering compassionate, patient-centered care with professionalism and accountability.
- Active Licensed Practical Nurse license in North Carolina or valid compact/multistate LPN license.
- Minimum 1 year of recent LPN experience preferred.
- Experience in LTC, Skilled Nursing, Rehab, Nursing Home, Med-Surg, or Acute Care support preferred.
- Current BLS certification from the American Heart Association preferred or required by facility.
- Ability to pass required background screening, drug screen, and credentialing.
- Strong communication, documentation, and patient-care skills.
- Long-Term Care / Skilled Nursing Facility experience.
- Rehab or Nursing Home experience.
- Experience with geriatric patient care.
- EMR documentation experience.
- Strong medication pass and patient monitoring skills.离
