Job Description

The Nurse Supervisor/Health & Wellness Coordinator (LPN/LVN) leads the assisted living and memory care community, overseeing nurse, caregiver and medication technician staff and ensuring high‑quality resident care.

• Manage day‑to‑day healthcare operations to ensure residents' healthcare needs are met.
• Ensure residents are treated with respect and dignity and quality care is maintained as needs change.
• Maintain consistent quality resident care and service delivery throughout all shifts, stepping into caregiver or medication tech roles as needed to meet staffing requirements.
• Supervise licensed nursing staff, medication technicians/CMA's and caregivers, providing training, education and schedule management.
• Oversee medication training, pharmacy management, medication supervision/administration; conduct periodic medication skills inventory checks and medication audits per Brookdale guidelines.
• Share on‑call duties as required.

• State nursing license (LPN/LVN or RN).
• Minimum 1–2 years relevant experience required.
