Work Schedule:
regular availability to work as needed on some weekdays, nights, and weekends; minimum requirement of two 8-hour shifts per month.
The License Practical Nurse (LPN) demonstrates competency in delivery of quality patient care to terminally ill patients ranging in age from infants to geriatrics. The LPN is responsible for implementing a plan of care developed by the Staff Nurse in accordance with the patient’s express wishes and in full compliance with the Nurse Practice Act and agency policy, procedures, and standards of care.
EssentialDuties
- Duties are delegated by and performed under supervision of a registered nurse
- Manage and organize schedule for the best route
- Make nursing visits and implement nursing activities consistent with the patient's Plan of Care. These include: administration of prescribed medical treatments, wound care, phlebotomy, medications, and basic bedside care. In addition, may irrigate, collect blood, change dressings; give medications/fluids through peripheral, PICC, midline, central venous access, and IVADs. May change dressings and sites, give medications/fluids through subcutaneous access catheters. May remove peripheral, PICC, midline, central venous catheter and subcutaneous catheters as assigned by RN.
- Focus visits (wound care/follow up/patient is declining)
- Communicate with team about changes with patient/schedule, etc.
- Collaborate/delegate as needed to Hospice Aides
In hospice care, you are dealing with grief. It is important to find ways to not take the heaviness of it all home with you. 1 year Clinical Nursing Experience
Licenses & Certifications- LPN
- Drivers License
- Preferred: CHPLN
