We are looking for a skilled LPN/RN to provide specialized care in home health settings. The successful candidate will treat patients according to the active plan of care, administer medication with precision, and document all activities meticulously. They will also educate patients and families, fostering understanding and empowerment.

• Provide specialized care: treat patients according to the active plan of care.
• Administer medication with expertise, ensuring patient comfort and safety.
• Document all activities, assessments, nursing actions, responses, and coordination of care.
• Educate patients and families on disease process, self-care techniques, and prevention strategies.

• Current North Carolina Licensed Practical Nurse License is required.
• Current BLS certification.
• Attention to Detail.
• Time Management Skills.
• Effective Problem-Solving and Conflict Resolution.
• Good Organization and Effective Communication.
• Home Health Experience (preferred).
