The Licensed Practical Nurse (LPN) demonstrates competency in the delivery of quality patient care to terminally ill patients ranging in age from infants to geriatrics. The LPN is responsible for implementing a plan of care developed by the Staff Nurse in accordance with the patient’s expressed wishes and in full compliance with the Nurse Practice Act and agency policy, procedures, and standards of care.
Essential Duties- Duties are delegated by and performed under the supervision of a registered nurse.
- Manage and organize the schedule for the best route.
- Make nursing visits and implement nursing activities consistent with the patient’s Plan of Care. These include: administration of prescribed medical treatments, wound care, phlebotomy, medications, and basic bedside care. May irrigate, collect blood, change dressings; give medications/fluids through peripheral, PICC, midline, central venous access, and IVADs. May change dressings and sites, give medications/fluids through subcutaneous access catheters. May remove peripheral, PICC, midline, central venous catheter and subcutaneous catheters as assigned by RN.
- Check vitals, weekly comprehensive visits.
- Focus visits (wound care/follow‑up). When patient is declining.
- May pronounce death.
- Document during visits.
- Communicate with team about changes to patient/schedule, etc.
- Collaborate/delegate as needed to Hospice Aides.
- Close‑knit team with available and supportive supervisors.
- Smaller company, visits are not so far apart.
- Independence, flexibility, and on‑call for 1 holiday a year, no weekends.
In hospice care, you are dealing with grief. It is important to find ways to not take the heaviness home. Many employees find that joy comes from being able to comfort patients, help them feel cared for, and make a difference during the time they have together.

- Associates or higher in Nursing.
- 1 year of clinical nursing experience.
- LPN license.
- Driver’s license.
- Auto liability insurance.
