Hospice of the Piedmont, Inc. seeks an RN Case Manager for a full-time, salaried exempt role. Schedule is Monday–Friday, 8am–5pm (40 hours/week). The nurse develops and implements patient-centered care plans aligned with patient wishes, adhering to Nurse Practice Act and agency policies.
Responsibilities include coordinating visits, documenting visits, and coordinating with the Nurse Practitioner and IDG. Requires NC RN license;
Associates degree required; BSN preferred.
